After back pain that I felt while bowling at the tail-end of last season carried forward into this season's pre-season nets, I decided I would go and get it checked out at a back physiatrist clinic. I took some notes of what he told me in my initial session as I found it very interesting. Don't know if it's of interest to anyone else, but it really opened my eyes to what I was doing with my body.

First of all we spoke about my pain while playing cricket, right back to when I first felt twinges at 15/16. Then began the battery of tests: He put me on scales, one for each foot, and I put 45kg of weight on my left leg, but just 30kg through my right. I also push my right shoulder forward and my left back and do the inverse with my pelvis in a relaxed stance, which causes a curve in my spine. There's a bias towards my left side that has led to a rib bulge, so when I bend over, one of my ribs pops out of my back and creates a lump. He took a photo and showed me and it's bizarre. I think I'm bending over normally trying to touch my toes, but you can see all of my left side, but the right just drops away, like it's barely doing anything. It was slightly disconcerting to bend over in front of another man as he stood behind me taking a photo, but needs must, ey? This also means the muscles in the right side of my back have over developed to compensate and the ones in the left side (where the pain is) are weak, though the inverse is true for my glutes and thighs.

I also have too little rotation in my ankle (I have 80 degrees and most people will have around 110), which also causes my knee to buckle slightly when I stride because it's trying to counter the problem. He asked me to do it with straight feet and I couldn't, I automatically splay my back foot and drop my knee. That's what it's like when I'm gently taking a long stride, so you can imagine what that process – at pace and with less control when I'm bowling – is doing to my joints. My left leg is also about 2cm longer than my right, which may be where the bias comes from. But all that is contributing to my lower back pain - he said the ankle part was key to sorting a lot of it out. I'm back next Monday to discuss my case and talk about managing it. He seemed hopeful that I could eliminate the causes of the pain and he isolated the muscle that was causing me the problem. He just went jabbing around my back asking if it was hurting and then hit it dead on, I nearly jumped off the bed. It was brilliant.

Sorry about the wall of text, but it was so good to see someone who didn't just say 'well you need to rest it', but tried to discover the underlying causes to the pain.

I'm not in bad shape. I'm not a fitness buff and don't go the gym, but I eat quite well (probably drink a touch too much, though). I also play squash quite regularly, which is good exercise, but I struggle to find someone to play against who can give me a decent game.

sounds excellent and very thorough!

A lot of cricketers get lower back pain. Myself and Ryan do on this forum. It would be interesting to hear from others on how they have managed back issues.

I pulled or popped something in my lower back helping to lift a sightscreen a couple of years ago. It stiffened up and I could barely make it out of bed the next few days. Eventually after a week or so I saw the Doctor who gave me some pretty heavy pain killers. Hey presto in 2 days I was back to my normal self. At least that's what I thought. Since then approx every 3/4 months it pulls and restricts my movement for a few days. I take some pain killers, go easy for a week or so and carry on. It has been steadily turning into a constant ache, making full movement particularly when batting difficult. Recently I purchased a massage seat and that seems to have helped alot as it does a lower back Shiatsu massage which gets into the muscle. It's not as good as a sports massage by a physio but my back has better movement and no episodes in the last 4 months. If it gets a bit sore, I spend 20 minutes or so on the massage chair and it feels much improved. Long term I'm trying to slim down and tone up, I'm carrying a few pounds and I'm sure it doesn't help.

Bah, too far. I've been looking for someone who is willing to look at it from all the points of view you've described, rather than just giving me a back massage or manipulate my pelvis a bit.

All I can say is avoid surgery unless you have no other options.

The best medicine is prevention.
I live on a regime of pilates and core stuff.

Oh, I've got 7 screws holding my lower spine together. I know this isn't a 'mines bigger than yours match', I mentioned it as I've had 13 ops and some experience.

Prevention, diet, training, warm-up, stretch, and then have a few beers.

What was interesting was that he told me that no injury should hurt for more than 6-12 weeks, as that's how long it takes for the body to heal. If it's more than that there are two reasons. Firstly, a repetitive, damaging action is constantly restricting the opportunity for the body to repair. Or, it's more nerve based, and the body identifies the area of weakness and makes it hypersensitive, meaning that things that didn't hurt before will now, because the nerves are more receptive to these signals. It was all very interesting, I was there for a good hour and a bit. I haven't had any treatment yet, but the first session was professional and encouraging.

ive had precisely the same problems 5 years ago and i've this week had my first tour without pain..  so ive had my injury in my back for 5 years.. and it only started getting better after i started getting my core stronger and kept going to my physio who massaged it lose and and cracked my back.  we worked on it together the strength and treatment i started a year ago and now i am basically pain free...

i have to agree : stretch, relaxation therapy, build it up, warm up and repetition.  and a good old massage around it is always a nice way to say sorry to your back ;-)

ohw and get your action checked see if it has any problems!

I usedto be able to bowl reasonably quick aged 12, but then I hit a growth spurt and shot up a foot in 6 months.  Never been able to find a repeatable, comfortable bowling action since.  I've started doing a lot of core work and pilates which has helped strengthen my lower back.  Having an osteopath, sports therapist and accupuncturist for a mother is also useful!

I have had mild lower back pain for a long time and was able to live with it than manage. The thought of going to a GP puts me off as I would be advised to take paracetamol for many weeks. I have been stretching regularly but the best thing I bought and used was a exercise foam roller from Argos for £ 10. I use it twice daily and my back feels much better. Foam rollers are quite versatile to use for stretching legs especially stiff quads and hamstring . Highly recommended.
