Because you were concentrating on just trying to play the straight drive your front foot went to the same spot every time, regardless of where the ball pitched. That meant you played around your pad for anything leg side, and sliced balls outside the off stump. Your technique is not bad, you just need to try and not commit you feet so early. It's funny how being low on confidence can affect the way your feet move, cricket can be such a head game.

Some very good advice on here and I'll try not to repeat what has already been said.

A few extra things I noticed is your back leg seems to collapse a little. Which could be a symptom of the balance/head issues already addressed, as well as your front foot stride length.

If you have a look at your top arm and where you're elbow finishes you'll see that you loose your shape. What that means is ideally you are aiming to create a diamond with your arms (see pic). This shape should remain throughout the shot right to the follow through. If this is achieved then your bat will come straight through the line of the ball and the shot will go where you want it to.

You will probably find though that if address your balance and head movement (not foot work, the feet follow the head) then the rest will come more naturally.

To practice start small, use tee drills, drop feeds, bobble feeds, throw downs, bowling machine, net bowling, middle practice, match.

Also there is no better person at this shot than Ian Bell. Check out this video. Not too heavy on the technical side of things, just a few key points, but the best thing is to just watch him, absorb what he does and try to recreate it.
